A simple snapshot of where you are—right now
This quick wellbeing check‑up is designed to help you pause, reflect, and gently take stock of how supported you feel across key areas of your life. There are no right or wrong answers, and this is not a test—it’s simply a moment of awareness.
You’ll rate eight dimensions of wellbeing using a scale from 1 to 10:
- 1 = this area needs attention or support
- 10 = you feel confident, resourced, and supported
Choose the number that best reflects your current experience, not where you think you should be or where you’d like to be someday. Go with your first instinct.
How to Complete the Check‑Up
- Review each dimension listed below the chart.
- Use the descriptions as a guide to choose your rating from 1–10.
- Your chart will visually reflect your responses, creating a snapshot of your overall wellbeing.
This exercise usually takes 5–10 minutes.
Tip: Aim for honesty over perfection. Awareness is the first step toward meaningful, sustainable change.
Dimensions of Wellness
- Physical Wellness: How would you rate your overall physical wellbeing? Consider your sleep habits, hydration, movement routines, daily energy levels, and the behaviors that support your physical health.
- Emotional Wellness: How aware are you of your stress levels, and how effectively do you use coping strategies and mindset shifts? Reflect on your emotional readiness for lifestyle change.
- Social Wellness: How strong and supportive are your relationships? Think about your support networks, accountability partners, and relationship patterns that influence adherence and motivation.
- Intellectual Wellness: How comfortable and motivated are you in exploring new ideas, problem‑solving, engaging in creative activities, or applying what you learn to daily life?
- Occupational Wellness: How well are you managing your work‑life routines, schedule demands, and time management? Consider how well your health habits align with your career responsibilities.
- Environmental Wellness: How supportive, safe, and healthy do you feel your surroundings are—at home, at work, and in the spaces where you spend most of your time?
- Financial Wellness: How confident are you in creating realistic, sustainable habits that fit within your financial resources? Do you manage your finances or do they manage you?
- Spiritual Wellness: How aligned do you feel with your values, purpose, and sense of meaning? Reflect on how these support short and long‑term life goals.
